Your Middle School student has been chosen to be a candidate for Leaders for the 21st Century!

For Three Days in Early June, over sixty young people who demonstrate leadership potential from Holland, Holland Christian, West Ottawa, Zeeland, Zeeland Christian, Black River, and Corpus Christi Middle Schools will have a unique opportunity to interact with local, regional, and national leaders.

Over a three day and a two-night stay at Hope College, your son or daughter will develop skills in team building, decision making and communication.

We look forward to meeting your student and having the opportunity to talk with you about this very special scholarship program, encouraged by your schools, and directed by your local Rotary Clubs. The entire cost of the conference has been paid by the Holland area Rotary Clubs, with the help of a grant from Rotary District 6290 and from local businesses who believe our future lies in these kids.

The following is important information regarding your child’s training. Please read and sign below.

All medications brought by the trainee (prescriptions or over the counter) must be given to a counselor at check in, except for inhalers. All medication must be in the original container and include clear and current directions along with the trainee’s name.

I understand it is the policy of Leaders for the 21st Century not to release a trainee to anyone other than the person designated at the beginning of the training.

I understand, too, that although the program has taken precautions to provide proper organization,
supervision, instruction, and equipment for each activity, it is impossible for the program to guarantee absolute safety.

I understand that my child shares responsibility for his/her safety, and I have instructed my child in the importance of knowing and abiding by training rules, regulations and procedures for the safety of the trainees. I understand that I will be responsible for picking up my child prior to the end of the training, if my child does not abide by the above stated rules. Further, I waive any claim that may arise against the Rotary Leaders for the 21st Century and its sponsors, the Rotary Clubs of Holland and Zeeland.

In signing this document, I hereby certify that the information concerning the Health Record is correct. I give permission for the release of medical information in case of illness or injury. I also give permission for Leaders for the 21st Century to use photographs taken of my son or daughter during the training for future Leaders publicity.
